Case management is at the heart of St. Vincent Haven's approach to helping men achieve lasting stability. Each resident is assigned a case manager who works with them to develop an individualized plan for achieving their goals.

Our case managers assist residents with:

• Employment search and job readiness
• Educational opportunities and GED preparation
• Mental health and substance abuse referrals
• Physical health care coordination
• Permanent housing search and placement
• Benefits enrollment (Medicaid, SNAP, Social Security)
• Financial literacy and savings planning
